Reporting Safety Incidents
Proper incident reporting helps identify problems, prevent future incidents, and maintain compliance with regulations.
What Should Be Reported
Report any of the following:
- Accidents: Any crash, collision, or damage
- Near-Misses: Close calls that could have resulted in an accident
- Equipment Failures: Any malfunction during operation
- Injuries: Any injury to people, no matter how minor
- Property Damage: Damage to third-party property
- Airspace Violations: Unintentional entry into restricted airspace
How to File a Report
- Navigate to Safety > Incident Reports
- Click "New Incident Report"
- Fill in all required information:
- Date, time, and location
- Type of incident
- Description of what happened
- Personnel involved
- Equipment involved
- Injuries or damage
- Attach any photos or evidence
- Submit the report
After Reporting
Once submitted:
- Your supervisor will be notified
- An investigation may be initiated
- You may be contacted for additional information
- Corrective actions will be tracked
FAA Reporting Requirements
Some incidents must be reported to the FAA:
- Serious injuries to people
- Significant property damage
- Airspace violations
