Reporting Safety Incidents

Proper incident reporting helps identify problems, prevent future incidents, and maintain compliance with regulations.

What Should Be Reported

Report any of the following:

  • Accidents: Any crash, collision, or damage
  • Near-Misses: Close calls that could have resulted in an accident
  • Equipment Failures: Any malfunction during operation
  • Injuries: Any injury to people, no matter how minor
  • Property Damage: Damage to third-party property
  • Airspace Violations: Unintentional entry into restricted airspace

How to File a Report

  1. Navigate to Safety > Incident Reports
  2. Click "New Incident Report"
  3. Fill in all required information:
    • Date, time, and location
    • Type of incident
    • Description of what happened
    • Personnel involved
    • Equipment involved
    • Injuries or damage
  4. Attach any photos or evidence
  5. Submit the report

After Reporting

Once submitted:

  • Your supervisor will be notified
  • An investigation may be initiated
  • You may be contacted for additional information
  • Corrective actions will be tracked

FAA Reporting Requirements

Some incidents must be reported to the FAA:

  • Serious injuries to people
  • Significant property damage
  • Airspace violations
